This includes diagnosis, prescriptions, medical certificates, medical abstract, laboratory results, results of annual physical exam (if applicable), confinement details and\/or medical consultation details.<\/span><\/li><li><span style=\"font-family: arial, helvetica, sans-serif;\">Government Issued Identifications (Passports, UMID, Drivers License, etc.)<\/span><\/li><li><span style=\"font-family: arial, helvetica, sans-serif;\">Company Profiles and Details<\/span><\/li><li><span style=\"font-family: arial, helvetica, sans-serif;\">Financial Details (Bank Account Information, etc.)<\/span><\/li><li><span style=\"font-family: arial, helvetica, sans-serif;\">Transaction History. This includes your communication with us but not limited to providers and other entities with regards to your insurance.
